By accessing our website, booking an appointment, or receiving medical care through our practice or affiliated entities, you agree to the following Terms & Conditions (“Terms”). Please read them carefully before using our site or services.

1. Overview

Arizona Sports Medicine PLLC is a sports medicine, regenerative medicine, and physical medicine practice based in Tempe, Arizona. We provide musculoskeletal care, ultrasound-guided procedures, regenerative therapies (including PRP, A2M, peptides, exosomes, and biologics), hormone optimization, physical therapy, and performance recovery services.
All care is provided by licensed healthcare professionals in compliance with applicable Arizona state law, federal medical regulations, and professional practice standards.

Your use of this website and our services signifies acceptance of these Terms and our Privacy Policy. If you do not agree, please discontinue use of the website and do not schedule services.

2. Affiliated Entities and DBA Notice

PROpeptides™ operates as an affiliated DBA (Doing Business As) under Arizona Sports Medicine PLLC.
All customers, clients, or patients purchasing or receiving evaluation, consultation, or peptide protocols through PROpeptides are considered patients of Arizona Sports Medicine PLLC.

This means:

  • All orders, telemedicine reviews, consultations, and medical clearances through PROpeptides are medically supervised by licensed providers under Arizona Sports Medicine PLLC.

  • All medical records and treatment authorizations are documented within Arizona Sports Medicine’s HIPAA-compliant electronic health record (EHR) system.

  • Any patient interaction, consent, or follow-up with PROpeptides falls under the clinical and legal oversight of Arizona Sports Medicine PLLC.

3. Medical Disclaimer

  1. Informational Use Only:
    The information provided on this website, including articles, videos, social media content, or product information, is for educational and informational purposes only. It is not intended as medical advice and does not replace professional diagnosis or treatment.

    Arizona Sports Medicine operates under the authority of the Arizona Medical Board and Arizona Naturopathic Physicians Board of Medical Examiners (depending on provider licensure).
    All services are rendered within the boundaries of Title 32, Chapters 13 & 14 of the Arizona Revised Statutes.

  2. Regenerative Medicine & FDA Oversight:
    Certain regenerative, peptide, or biologic therapies may be classified as off-label or investigational under current FDA guidance. These services are provided only after appropriate informed consent and acknowledgment of their investigational status.
    We do not offer or promote any therapy in violation of A.R.S. § 13-3731, FDA regulations, or federal biologic product laws.

  3. Advertising & Consumer Protection:
    Arizona Sports Medicine adheres to the Arizona Consumer Fraud Act and does not make false, misleading, or exaggerated claims regarding medical outcomes or performance enhancement.

5. Patient Responsibilities

By scheduling or receiving care, you agree to:

  • Provide complete and accurate health, medication, and treatment history.

  • Follow pre- and post-procedure instructions as directed.

  • Communicate any changes in health status or complications promptly.

  • Understand that some services are not covered by insurance and may require self-payment.

  • Conduct yourself respectfully toward staff and providers.

  • Refrain from recording, distributing, or posting any clinic-related materials without written authorization.

Failure to comply with these expectations may result in termination of services at our discretion.

6. Payment, Insurance & Refund Policy

  1. Payment Terms:
    Payment is due at time of service, unless otherwise specified in writing.
    Certain procedures (e.g., PRP, A2M, Tenex, peptide programs, hormone optimization, and physical therapy packages) may require prepayment or deposits.

  2. Insurance Coverage:
    Arizona Sports Medicine accepts select insurance plans for covered services.
    Regenerative, peptide, and performance therapies are cash-pay and non-reimbursable under most insurance plans.
    A detailed invoice or “superbill” can be provided upon request for out-of-network submission.

  3. Refunds & Cancellations:

    • Refunds are reviewed on a case-by-case basis.

    • No refunds are provided once a treatment, injection, or service has been performed.

    • Appointments cancelled with less than 24-hour notice may incur a cancellation fee.

    • Prepaid memberships or multi-session programs are non-transferable and non-refundable after initiation.

  4. Collections:
    You agree to pay any outstanding balances, applicable interest, or collection costs as permitted under Arizona law.

7. Privacy & Confidentiality

  1. HIPAA Compliance:
    Arizona Sports Medicine and PROpeptides comply fully with the Health Insurance Portability and Accountability Act (HIPAA) and Arizona Revised Statutes §12-2291–2297.
    Medical records are encrypted, stored securely, and accessible only to authorized personnel.

  2. Data Use:
    Your information may be used for treatment, billing, and operational purposes consistent with HIPAA. We do not sell or distribute patient data.

  3. Electronic Communication:
    By providing your contact information, you consent to receive secure emails, text messages, or patient portal communications. You may opt out at any time.

8. Website Use & Intellectual Property

All website content — including text, media, trademarks, and logos — is the property of Arizona Sports Medicine PLLC or its affiliates and protected under U.S. and international copyright law.

9. Limitation of Liability

To the fullest extent permitted by law:

  2. Any dispute or claim arising from these Terms or services rendered shall be resolved by binding arbitration in Maricopa County, Arizona, in accordance with the American Arbitration Association.

  3. You waive any right to trial by jury or participation in class actions related to these Terms or services.

11. Modifications to Terms

We may update these Terms periodically. Revised versions will be posted on our website with an updated “Last Revised” date. Continued use of our website or services constitutes your acceptance of the updated Terms.
