Our Philosophy: Treat the Patient, Not Just the Number

Hormone medicine should never be reduced to a single testosterone result.

Current endocrine guidelines emphasize that testosterone deficiency should be evaluated using both clinical symptoms and appropriately obtained laboratory testing. Testosterone concentrations also fluctuate, which is why repeat morning testing may be appropriate when establishing a diagnosis.

At Arizona Sports Medicine, we combine:

  • Clinical symptoms and medical history

  • Comprehensive laboratory testing

  • Total and free hormone concentrations when appropriate

  • Pituitary and reproductive hormone signaling

  • Thyroid and metabolic health

  • Blood counts and cardiovascular risk markers

  • Fertility and family-planning goals

  • Medications and other factors capable of suppressing normal hormone production

  • The patient's goals and response to treatment

We then monitor patients at routine intervals to determine whether treatment is producing the desired clinical response while maintaining hormone levels within appropriate physiologic ranges.

The objective is optimization of normal physiology—not chasing the highest possible laboratory value.

Low Testosterone Is More Than a Number

Testosterone plays an important role in far more than sexual function.

In men, testosterone contributes to:

  • Sexual desire and erectile function

  • Muscle mass and strength

  • Bone mineral density

  • Red blood cell production

  • Energy and vitality

  • Mood and cognitive function

  • Body composition

  • Recovery from exercise

  • Reproductive function and sperm production

Symptoms associated with testosterone deficiency can include reduced libido, erectile dysfunction, decreased spontaneous erections, fatigue, loss of muscle mass, difficulty recovering from exercise, changes in body composition, reduced motivation, irritability, difficulty concentrating, depressed mood, infertility, and decreased bone density.

Many of these symptoms are nonspecific. Poor sleep, obesity, excessive caloric restriction, medications, chronic disease, stress, metabolic dysfunction, and other endocrine abnormalities can produce similar complaints.

That is precisely why a testosterone prescription should not be the first step in every patient with fatigue or decreased performance.

The first step should be understanding the patient.

The ADAM Questionnaire

One tool that may be used during the evaluation of androgen deficiency is the Androgen Deficiency in the Aging Male—or ADAM—questionnaire.

The questionnaire asks about symptoms commonly associated with androgen deficiency, including changes in:

  • Libido

  • Energy

  • Strength and endurance

  • Enjoyment of life

  • Mood

  • Erectile function

  • Exercise performance

  • Work performance

  • Daytime sleepiness

The ADAM questionnaire can be useful as a screening and discussion tool, but it is not a diagnostic test.

Research has found that the questionnaire can be relatively sensitive for identifying men who may warrant further evaluation, but its specificity is limited. Many symptoms appearing on the questionnaire can occur for reasons unrelated to testosterone.

For that reason, we do not diagnose testosterone deficiency from a questionnaire alone.

Symptoms initiate the investigation. Laboratory testing helps define the physiology.

Testosterone Levels Have Been Changing

Testosterone does tend to decline with age, but aging alone does not explain everything being observed.

One frequently cited population study by Travison and colleagues examined American men over time and identified a population-level secular decline in testosterone concentrations that could not be explained simply by increasing age.

This is an important distinction.

A 35-year-old man today should not automatically be assumed to have healthy testosterone physiology simply because he is “too young to have low testosterone.”

At Arizona Sports Medicine, we do not use age as the deciding factor for whether someone deserves an evaluation.

We increasingly see younger adults presenting with symptoms potentially consistent with hormonal dysfunction.

That does not mean every younger man needs testosterone.

It means he deserves the same careful evaluation as anyone else.

Sleep, obesity, metabolic health, medications, nutrition, excessive training, chronic illness, stress, reproductive signaling, and other potentially reversible factors must also be considered before deciding how—or whether—to intervene.

Hormone Therapy Has Carried a Historical Stigma

Hormone replacement has had a complicated medical history.

For years, testosterone therapy was surrounded by concern regarding cardiovascular disease and prostate health. In 2015, the FDA required testosterone labeling to include information about a possible increased cardiovascular risk.

Evidence continued to evolve.

The large TRAVERSE cardiovascular safety trial subsequently evaluated more than 5,000 men with hypogonadism and cardiovascular disease or cardiovascular risk factors. Based partly on those findings, the FDA announced in 2025 that the boxed-warning language concerning increased cardiovascular outcomes should be removed from testosterone products, while adding or strengthening warnings regarding increases in blood pressure.

That does not mean testosterone is risk-free.

It means modern hormone medicine should reflect modern evidence rather than either extreme—neither treating testosterone as inherently dangerous nor treating it as a harmless performance supplement.

The appropriate approach is diagnosis, individualized treatment, physiologic dosing, risk assessment, and ongoing monitoring.

Women's Hormone Therapy Has Evolved Too

Few areas of medicine demonstrate the changing understanding of hormone therapy better than women's health.

The publication of the Women's Health Initiative results in 2002 dramatically changed public and medical attitudes toward menopausal hormone therapy. Hormone prescriptions declined substantially, and estrogen and progesterone therapy became associated with considerable fear.

Subsequent analysis has provided a more nuanced understanding of the findings, including differences related to patient age, timing of therapy, formulation, indication, and individual risk.

In February 2026, the FDA approved labeling changes for several menopausal hormone therapy products removing boxed-warning statements related to cardiovascular disease, breast cancer, and probable dementia, reflecting this evolving assessment of the evidence.

Hormone therapy still requires appropriate screening and individualized risk assessment.

But women should also have the opportunity to discuss their symptoms and treatment options based on current evidence rather than decades-old generalizations.

Hormone Optimization for Men

We Don't Automatically Start With Testosterone

One of the most important distinctions in our program is that low testosterone does not automatically mean injectable testosterone.

Exogenous testosterone can suppress the hypothalamic-pituitary-gonadal axis—the signaling system responsible for stimulating the testes to produce testosterone and sperm.

That matters.

For a 32-year-old man who wants children, the treatment discussion may be very different from that of a 62-year-old man with established hypogonadism who has completed family planning.

Before recommending replacement therapy, we consider:

Is testosterone actually deficient?

Are symptoms consistent with the laboratory findings?

Why is testosterone low?

Can contributing factors be corrected?

Is maintaining endogenous testosterone production important?

Does the patient want children now or in the future?

Only then do we determine the most appropriate pathway.

Fertility Matters

Testosterone and fertility are closely connected—but taking testosterone and producing sperm are not the same thing.

Normal intratesticular testosterone concentrations are important for spermatogenesis. Exogenous testosterone can suppress LH and FSH signaling from the pituitary and substantially decrease sperm production, sometimes producing azoospermia.

For this reason, major endocrine guidelines recommend against beginning conventional testosterone replacement in men actively planning fertility.

We specifically discuss family planning before initiating therapies capable of suppressing endogenous production.

Depending upon the patient's physiology, goals, and medical appropriateness, treatment may instead focus on addressing reversible causes of suppression or considering strategies intended to support endogenous hormonal signaling.

Hormone therapy should fit your life plan—not interfere with it.

Injectable and Oral Treatment Pathways

There is no single hormone protocol appropriate for every patient.

When replacement is indicated, we offer different treatment pathways based on the patient's laboratory findings, symptoms, preferences, response, and goals.

These may include:

Injectable Therapy

Testosterone injections can provide predictable delivery and allow individualized dosing. Patients who choose injections receive education regarding preparation, injection technique, dosing, supplies, and safe administration.

In-office injections are available at no additional administration charge for established hormone patients.

Patients who prefer home treatment can be taught to self-administer injections or have a spouse or partner assist them.

Oral and Non-Injectable Strategies

Not every patient needs—or wants—to begin with injectable testosterone.

Where medically appropriate, we can discuss oral or other therapeutic strategies and approaches intended to address endogenous hormonal production rather than immediately replacing it.

The appropriate pathway depends on the underlying physiology and treatment objective.

Many patients come to us already receiving testosterone or other hormone therapies from another clinic.

Sometimes they are doing well.

Sometimes the dosage has gradually increased.

Sometimes multiple medications have been added to control side effects from other medications.

And sometimes the patient simply no longer understands why they are taking everything they have been prescribed.

We can independently evaluate the regimen and determine what still makes sense.

For patients interested in reducing or discontinuing testosterone or other hormone therapies, we can develop a monitored transition strategy when medically appropriate.

The goal is not to keep someone on medication indefinitely simply because another clinic started it.

The goal is to determine what that individual actually needs.

Testosterone Therapy for Women

Testosterone is not exclusively a male hormone.

Women naturally produce testosterone and other androgens throughout life,

although at substantially lower concentrations than men. Testosterone physiology

can influence sexual function and other aspects of health.

The evidence for testosterone therapy in women, however, requires an

important distinction.

The strongest international consensus evidence supports carefully dosed physiologic

testosterone therapy for appropriately evaluated postmenopausal women with

hypoactive sexual desire disorder (HSDD) after other contributing factors have been considered.

Evidence is not currently strong enough to claim that testosterone universally improves energy, cognition, muscle mass, weight loss, or general well-being in all women.

That distinction is important to us.

We evaluate testosterone as one component of female hormone physiology, rather than treating every symptom with testosterone.

Estrogen, Progesterone & Female Hormone Optimization

For women, hormonal symptoms may involve interactions among estrogen, progesterone, testosterone, thyroid function, reproductive signaling, metabolic health, sleep, and the menopausal transition.

Potential symptoms prompting evaluation may include:

  • Hot flashes or night sweats

  • Changes in menstrual patterns

  • Sleep disruption

  • Sexual dysfunction

  • Vaginal or genitourinary symptoms

  • Changes in mood

  • Changes in body composition

  • Loss of bone density

  • Symptoms occurring during perimenopause or menopause

When appropriate, treatment options may include estrogen, progesterone, testosterone, or a combination, depending upon the patient's symptoms, reproductive status, anatomy, laboratory evaluation, risk profile, and treatment goals.

For example, systemic estrogen therapy in a woman with an intact uterus generally requires consideration of adequate progestogen protection of the endometrium.

Female hormone therapy is therefore not simply a matter of “balancing three numbers.”

It requires individualized medical management.

Comprehensive Hormone Testing

We Look Beyond Testosterone

A basic testosterone measurement can tell us something.

A comprehensive panel can tell us much more.

Depending upon the clinical situation, evaluation may include assessment of androgen status, estrogen balance, pituitary signaling, thyroid function, metabolic health, blood counts, organ function, cardiovascular risk, nutrient status, and other markers relevant to symptoms and treatment safety.

The exact panel is individualized because a 28-year-old athlete concerned about testosterone and fertility should not necessarily receive the same evaluation as a 55-year-old woman experiencing menopause.

Wellness Panel

Our Wellness Panel is designed to provide a broad baseline assessment of health rather than looking at one isolated hormone.

It can be useful for patients who want to understand their overall physiology, establish baseline values before treatment, or investigate symptoms that may have multiple contributing factors.

The objective is to evaluate hormone health in the context of the systems that influence it—including metabolic, hematologic, thyroid, cardiovascular, and general health markers.

This gives us a much better foundation for making treatment decisions than a testosterone value alone.

Performance Panel

Our Performance Panel provides a more detailed evaluation for patients interested in recovery, body composition, training capacity, hormone function, and performance.

This can be particularly useful for athletes, highly active adults, patients experiencing unexplained changes in recovery or body composition, and individuals considering hormone or peptide-based optimization strategies.

Rather than asking only:

“Is my testosterone low?”

we want to understand:

“What does the larger physiologic picture look like, and what factors can we actually improve?”

We Make Laboratory Testing Accessible

Laboratory monitoring should not become an unnecessary barrier to appropriate medical care.

Whenever possible, we bill medically appropriate laboratory testing through insurance.

For patients without coverage, with high deductibles, or who prefer self-pay testing, we also offer competitively priced comprehensive laboratory panels.

Established patients can also have their laboratory specimens collected in our office.

This allows evaluation, treatment, monitoring, and follow-up to remain connected within the same clinical program.

Natural Hormone Optimization Still Matters

Medication is only one component of hormone health.

Sleep quality, resistance training, adequate energy intake, body composition, micronutrient status, alcohol consumption, metabolic health, stress, medications, and recovery can all influence endocrine function.

When appropriate, we may incorporate:

  • Nutrition strategies

  • Exercise and resistance-training recommendations

  • Sleep optimization

  • Weight and metabolic management

  • Evidence-informed supplementation

  • Correction of identifiable nutritional deficiencies

  • Recovery strategies

  • Peptide therapies when medically appropriate

For some patients, improving these variables may improve hormone physiology without immediately beginning replacement therapy.

For others, these strategies become part of a comprehensive program alongside medically indicated hormone replacement.

Hormones + Peptide Medicine

Arizona Sports Medicine also offers medically supervised peptide therapy, allowing us to consider hormone optimization within a broader recovery and performance framework.

Peptides are not substitutes for deficient hormones.

Instead, selected peptide therapies may be considered for appropriate patients as part of individualized strategies involving recovery, body composition, metabolic health, sleep, or other specific clinical objectives.

We do not believe in simply stacking medications.

Every therapy should have a reason for being there.

Routine Monitoring Is Part of Treatment

Starting hormone therapy is not the end of the process.

It is the beginning of monitoring.

Depending upon the therapy and patient, follow-up may include assessment of:

  • Clinical symptom response

  • Testosterone concentrations

  • Estradiol when indicated

  • Hemoglobin and hematocrit

  • PSA and prostate considerations when appropriate

  • Blood pressure

  • Lipids and metabolic markers

  • Liver and kidney function

  • Female reproductive hormones when relevant

  • Fertility considerations

  • Medication side effects

  • Other individualized safety markers

If laboratory values or symptoms indicate that treatment should be modified, we modify it.

The objective is to find the lowest reasonable treatment burden capable of producing an appropriate clinical response and physiologic hormone concentrations.

One is to dismiss meaningful symptoms because a laboratory value technically falls inside a reference interval.

The other is to prescribe aggressive hormone therapy to nearly everyone who walks through the door.

We believe there is a better approach.

Listen to the patient.

Measure the physiology.

Investigate the cause.

Correct what can be corrected naturally.

Consider fertility and long-term goals.

Use medication when it is appropriate.

Monitor the response.

Adjust when necessary.

And never forget that the objective is not to create the highest testosterone number on a laboratory report.

The objective is to help people feel and function like themselves again.

Important Medical Information

Hormone therapy is not appropriate for every patient. Evaluation, laboratory testing, contraindication screening, and ongoing monitoring are important components of treatment. Benefits and risks vary according to the specific hormone, formulation, dose, age, medical history, reproductive goals, and other individual factors.

Testosterone therapy can suppress sperm production and endogenous testosterone production. Patients interested in future fertility should discuss reproductive goals before beginning treatment.

The ADAM questionnaire is a screening instrument and does not independently diagnose testosterone deficiency. Current endocrine guidance supports establishing male hypogonadism through compatible signs or symptoms together with consistently low, appropriately measured testosterone concentrations.

Testosterone therapy in women requires a separate clinical framework from male testosterone replacement. Current international consensus evidence most clearly supports physiologic testosterone therapy for appropriately evaluated postmenopausal women with hypoactive sexual desire disorder; evidence for other indications remains less established.

Hormone and peptide therapies should only be used when medically appropriate following individualized evaluation.
